Rationale for Mobile Medical Clinic.
The Mobile Medical Clinic (MMC) concept was introduced by Ghana Ex-Servicemen Association Hamburg e. V (GESAH) in 2020 in cooperation with Meine Diaspora e. V. and YAPA Germany e.V to help provide healthcare to underserved African community and other minority communities in Hamburg. With sponsorship from BürgerStiftung Hamburg the concept was magnificently launched in September 2021 at Horner Rennbahn Hamburg under the program African Community Strategic Leadership meeting. The meeting developed a standardized framework for engaging with African communities and the potential stakeholders in Hamburg. The meeting also documented and outlined the services that were going to be provided at the Mobile Medical Clinic and explained the roles and responsibilities for each stakeholder. Some of the key strategic areas of the intervention included:
• Eliminating structural barriers e.g. uninsured.
• Community health education and health promotion.
• linking community access to primary healthcare.
• Healthy eating and active living. This improves diet and increase physical activities. It reduces rates of childhood obesity and adult obesity

Solutions We Offer
The MMC will travel to medically under-served areas to help connect patients to health care when patients would have otherwise gone without care. MMC fills the void left by an absence of reliable care partners that fit the African community. It will serve as a comfortable entry point to the health system for patients who may be disengaged or distrustful of the health care system.
It has become increasingly relevant as a growing population of patients in the African community finds themselves without a network of support. It will reduce the burden of logistical barriers (e.g. work hours and failed many check-up appointments) The Mobile Medical Clinic will provide good health benefits to disadvantaged and low-income African communities across the state of Hamburg by providing preventive care (health screening), primary health care: i.e. Blood pressure: Weight Checks: Immunizations: Cholesterol Screening:

The proposed clinic will be going out on rotation at least five times a week (Wednesday, Thursday, Fridays, Saturdays and Sundays) to Churches/mosque premises (Fridays) after religious services and provide them with appropriate care in a nice and clean clinical setting (Mobile Facility). In this regard there are over 100 African churches in the community MMC will bring healthcare access to patients by pulling right into a patient's neighborhood making the experience a little more trustworthy and convenient with staff who are easy to talk to and also can understand them. The community members will feel that the clinics are reaching out to care about them, thereby inspiring them to take charge of their own health.

The MMC will serve the medically disenfranchised African community members – individuals who are under-insured e. g in the dental care, uninsured, or who are otherwise disconnected from the healthcare system due to access barriers. Many of the non-elderly uninsured patients lack a usual source of healthcare. The disenfranchised, those without adequate healthcare and under-served populations lack fundamental awareness of the availability of healthcare.
- Together, we can develop a mobile health clinic to serve the residents of African community Hamburg by January 2023
- To foster community involvement in healthcare: with appropriate referrals and follow-up appointments for every patient.
- To deliver culturally respected and linguistically sensitive health promotion, disease management and health education with comprehensive care plans for every patient.
- To provide healthcare services to every patient regardless of insurance or ability to pay.
- To be fully operational and interacting with people at convenient locations with a full patient load by July 2023
- Our Mobile Medical Clinic, will be in the field to bring health care services closer to the people and our services will be carried out by highly trained professional doctors, dentist, opticians, nurses, nurse’s aides, mental health counsellors, medication management counsellors, physical therapists and other health and non – health workers, who know what it takes to provide medical services better. The intended package of services included a range of comprehensive Primary Care Services which include:
- Preventive screening e.g. Blood pressure: Weight Checks: Immunizations: Cholesterol Screening: Vision Screening: Glucose: Wellness Education
- Preventive screenings, health education, referrals to social services and community health centres to address patients' highest needs. Referral to other programs will be addressed e.g. Speciality clinics, Emergency care for additional care (e.g. X-ray, Prescriptions)
- Clinical care (e.g physicals, immunizations, chronic illness management, behavioural health support, urgent care) legal aid and social services. Additional prevention services included health education on cervical cancer. Special emphasis will be placed on early diagnosis through the identification of symptoms and signs. Additional screening services will be provided for other illnesses through early diagnosis based on symptoms and signs
- Pharmaceutical connection program: to connect Pharmacy to supply medications free of charge or reduced-cost medication by searching the database of patient assistance Programs to enable patients start prescriptions immediately.
- MMC program will address needs of homeless patients during and after visit. It will focus on eliminating barriers that impede access to care and medication by connecting them to community organizations / agencies serving homeless population.
- The project will offer a stable community health platform through its partnership with Deutsche Rote Kreuz (DRK) Diakonie / Caritas, Fire Service, ASB etc.
- We will reach under-served populations by providing free and low cost medical care
- Will reduce overall Emergency Room visits and hospital admissions by our patients
- a) Our assumption of General funding, when approved will come from philanthropic support, state and federal funding, grants/foundation support, and independent /drug companies and private insurance providers. b) No Patients will be asked to pay consultation fee or charges.
